One of the most important duties of engineers is the investigation of technical failures, and proposing solutions so that they do not occur again. All plants and equipment have one thing in common - at some point in their life they will fail. Often the failure results in major losses to the organisation – of people’s lives, health, money, or reputation! So it needs to be investigated – to ensure that it never happens again. But can you do this properly?
The real Root Cause must be determined, and an effective corrective action implemented. This requires commitment from top management and a powerful Incident Management Strategy. This course shows the requirements for a really good Incident Management system, and teaches engineers how to carry out an excellent investigation, so that they can assure management that “it will never happen again!”
Fascinating case studies of actual major incidents like the Titanic, Concorde, and Chernobyl disasters are used to illustrate the approach required for effective investigation. Several excellent Root Cause analysis techniques are shown, including Fish-bone, 5 Why’s, and the Why Staircase.
